Below is a summary of the key responsibilities of this position:

**Coordinate with multidisciplinary team members in the coordination of patient care at home.
**Meet and evaluate perspective new home patients and explain modality options and home dialysis requirements.
**Be a liaison between the patient, supply company, PCP, and transplant to ensure continuity of care.
**Become proficient in the understanding and documentation of dialysis equipment and water quality in the home.
**Participate in care plan meetings, charting to maintain complete and accurate records.
**Interact with patients and their families to provide instruction and assess daily patient care needs and communicate patient needs to direct supervisor/physician.
**Participate in patient office visits with physician. Accurately receive and implement new physician orders.
**Schedule, obtain, review, and communicate results of tests to physician. Relay order changes to patient and caregiver.
**Complete required clinic records.

Education/Experience:
*Graduated from an accredited Registered Nurse program with current
*Alaska license or eligibility to obtain an Alaska license.
*Dialysis experience preferred.
*BSN preferred.
*Previous teaching experience or case management experience preferred.

Home Hemodialysis Patient Care Technician - Dimond
Patient Care Technician

We are seeking an energetic, dedicated person to fill a Patient Care Technician position at our dialysis clinic in Anchorage, AK.

The primary responsibilities of this position are:

**Cares for and monitors patients before, during, and post dialysis treatment.
** Prepares dialysis machine and it's components for treatment.
** Transports patients, records patient's weight, temperature, blood pressure, pulse rate and respiration rate.
** Explains dialysis procedure and operation of hemodialysis machine to patient before treatment to allay anxieties.
** Inspects and cleans area of access (fistula, graft, or catheter)
** Inspects equipment settings, including pressures, conductivity (proportion of chemicals to water), and temperature to ensure conformance to safety standards.
** Starts patient treatment according to physician order. 
** Monitors patient for adverse reaction and hemodialysis machine for malfunction.
** Assists in inventory and supply stocking in the dialysis clinic. 

Experience & skills needed for success:

** 1+ year of dialysis experience preferred, some health care/direct patient care experience required
** High school graduate, GED, or equivalent required
** CPR certification desirable -- must have ability to become certified
** Ability to become nationally certified within required time frame
** Strong communication and interpersonal skills
** Knowledge of computers and technical skills
** Enthusiastic team player with a strong work ethic

Registered Nurse - InCenter  Dimond Clinic

Our growing dialysis clinic is seeking an Alaska licensed RN (or an RN with the ability to license in AK) to perform the following:

***Responsible for the direct and indirect nursing care provided for a primarily adult/geriatric ESRD patient population.
***Utilizes nursing process to assess individual patient needs, determine and implement a plan of care, evaluate and revise plan according to changing patient care needs.
***Initiates and provides on-going patient and family education and includes patient/family in care planning, using appropriate forms to document care provided.
***Maintain communication with physicians, ancillary services, and other healthcare providers to ensure continuity of care for each patient.
***Responsible for maintaining standards of professional nursing practice and delegating tasks appropriately.

Requirements:

***Graduate of an accredited school of nursing: A.D.N. or nursing diploma required. BSN is preferred.
***1 year of nursing required, dialysis experience preferred.
***Leadership ability and organizational skills.
***Computer software knowledge.
***Ability to communicate effectively.
***Ability to understand and implement government standards.
